Usually urinary fistula after a surgery does not need treatment.

سیستم دفاعی بدن در مقابل عفونت ها که عامل پاتوژن آنها میکروبها، ویروسها، کلامیدیاها، قارچها و سایر میکروارگانیسم ها می باشند باید مقاومت کنند وگرنه انسان هر روز و هرساعتی دچار عفونتهای گوناگون می شود، شدت و حدت بیماری نیز بستگی مستقیم با عامل پاتوژن و قدرت دفاع سیستم ایمیونیتی (مقاومت دفاعی بدن) دارد.

 اکثراًعفونتها با علائمی نظیر تب اگر حاد باشد و بیمار شخصی باشد که Excess باشد و گاهی عفونتهای بدون تب اگر شخص deficiency باشد. و زمانی همراه با تب و لرز توأم می باشد. از اینجا اصطلاحات TCM وارد معرکه شد Excess  چیست و deficiency کدام است در Excess  به همان گونه که عامل پاتوژن فعال و نانجیب است عامل دفاعی بدن هم حریف و درگیر است و این جنگ و ستیز با شدت ادامه دارد اینجا تب و بی قراری غوغا می کند .

در deficiency نه عامل پاتوژن زیاد فعال است نه عامل دفاعی و عامل دفاعی اگر چه فعال نیست ولی جلوی آن ایستاده ولی جنگ و ستیز شدت و حدت ندارد تب یا نیست یا کمی است . در هر دو صورت فرقی نمی کند یعنی چه Exc یا Def هر دو نیاز به درمان دارند Exc و Def چیزی به نفع بیمار نیست . سیستم ایمینیتی هم با این وضع کم کم عادت می کند یعنی تقریباً این وضع را تحمل می کند این وضعیت را در طب غرب می گویند بیماری مزمن شده و حاد نیست . در بیماری های مزمن نه آن است که جنگ فیصله پیدا کرده و صلح شده بلکه جنگ ادامه دارد ولی شدت و حدت آن زیاد نیست کما اینکه از محل عفونت چرک خارج می شود یعنی گلوبولهای سفید کشته شده و اجساد میکروب ها هردو تلفات می دهند مثلاً بیمار سرفه می کند خلط زرد چرکی هم دارد اما تب یا نیست یا کم است . حالا در مورد بیماری فکر کنید که در ICU سه یا چهار نوع آتی بیوتیک تزریقی می گیرد ولی تب 5/38 درجه ای ذره ای کم نمی شود این بیماری عفونی Acute  حاد است . آیا آنچه طب سوزنی چین Exc یا Def نام برده همان ویرولانس میکروبی است؟ ویرولانس یعنی شدت بیماری زایی میکروب یا میکروبی که از پریتوان خوکچه هندی چند بار عبور داده شده و به شدت فراگیر و وخیم و بیماری زا است مثل میکروبهای جنگی که در محیط تکثیر و کشت آنها همه نوع آنتی بیوتیکی هم وارد می کنند تا ویرولانس را بالا ببرند . چه باید کردآیا آنتی بیوتیکهای باکتریواستاتیک و یا باکتریوسید چاره کار است؟

ببینیم TCM (طب سنتی چین) در این وضع چه کار میکند

بله TCM ضربه ای به سیستم دفاعی بدن که اینک خفته است می زند و آنرا بیدار و برانگیخته می کند.در این حال یک دکتر مدیکال آکوپنگچر(پزشکی که هم تخصص طب غرب را دارد وهم آکوپنکچر می داند) این نقاط را سوزن می زند :

Stomach 36 ، Large intestine 4 ، Large intestine 11 Govern vessel 14

توضیح : پیدا کردن این نقاط در بدن مستلزم دانستن آنوتومی سطحیSurface Anatomy  است و سپس دانستن تکنیک استخراج QI

(تلفظ کنید چی) می باشد حتی 2 تا 3 میلیمتر پایین و بالا زدن نقطه آکوپوینت (محل سوزن زدن) زمین تا آسمان تفاوت دارد. این نقاطی است که سیستم دفاعی بدن را برانگیخته می کند  و تعادل به هم خورده Yin , Yan را تنظیم می کند.

همانطور که قبلاً گفتیم اینجا عامل عفونت تعادل Yin , Yan را به هم زده  در حالت سلامت 1= Yin ×  Yan این همان معادله xy=1 است ومحاسبه   اگر منحنی جبری آن را رسم نماییم تصویر به دست آمده که در آن نقطه H نقطه تعادل و Healthy  بوده و دراین منحنی هر چه به طرف بالا برویم بیمار مبتلا به yin deficiency و هر چه روی منحنی رو به پایین می آییم بیمار دچار yan deficiency  میگردد yan def , yin def در واقع تشکیل زیر زمینه های بیماری هستند . بعد از انجام آکوپینچر یا طب سوزنی در این نواحی تب فرو می افتد و عفونت درمان می گردد در عفونت های ریوی بیمار می گوید خلط زرد من سفید وآبکی شد در اوستئومیلیت می گویند دیگر چرک نمی آید بلکه (جمله عامیانه بیمار) بلکه آب خالص می آید .در زخمهای دیابتی پاشنه و کف پا بیمار می گوید دیگر بد بونیست یعنی عفونت بی هوازی یا هوازی در کار نیست در مننژیت و آنسفالیت بیمار از حالت آپاتی درآمده و شناسایی پیدا کرده و تب فرو می افتد . در ساده ترین عفونت ها(آنژین چرکی) بیمار بهبود می یابد اما … اما در مورد همین آنژین چرکی هنوز اگر کسی از لیوان این بیمار آب بخورد مبتلا به آنژین چرکی خواهد شد پس میکروب از بین نرفته بلکه سیستم دفاعی این بیمار دو صد چندان افزایش یافته . اما در مورد اینکه این سوزنها و این نقاط چه مکانیزمی داشته اند که بیماری ها را معجزه آسا درمان می کنند باید پرسید آیا این مکانیزم ها تقلیدی از آنتی بیوتیک های باکتریوسید یا باکتریواستاتیک دارند؟ آیا واقعاً سلولهای دفاعی بدن تقویت شده اند؟ آخر چه مکانیسمی؟ چطور؟

تحریک Li11 نابود کننده تب است و آنچنان هیبت تب را فرو می ریزد که انسان تعجب می کند نه بعنوان آنتی آنفلاماتورهای غیر استروئیدی مثل آسپرین بلکه عفونت را از بیخ و بن می کند .

تحریک این نقطه باید با تکنیک Reduce انجام گیرد وگرنه به تب بیمار افزوده می شود تکنیک Reduce آن است که سوزن را در این نقطه می زنیم و از بیمار سوال می کنیم آیا برق ترا گرفت اگر گفت بله نقطه صحیح است (استخراج QI)  اما تکنیک : سوزن را تند فرو می کنیم و یواش و با چرخاندن آن را بیرون آورده و دو میلیمتر مانده دوباره تند داخل و آهسته خارج می کنیم

DU 14 یکی از نقاط بسیار مهمی برای درمان عفونتهای سیستمیک می باشد. و این همان نقطه است که اشخاصی که حجامت می کنند نادانسته و بدون اطلاع از خواص آن روی آن حجامت و یا به اصطلاح طب چین Blood letting  می کنند . محل DU 14 دقیقاً زیر زائده spinous process هفتمین مهره گردنی تقریباً همسطح با شانه ها می باشد. اصولا حجامت در طب چین یعنی خارج کردن اتش از بدن که در بیماریهای تب دار این کار انجام میگیرد. Bladder 40  هم یکی دیگر از نقاطی است که برای دفع  Heat یا عفونت بکار میرود درست وسط popliteal قرار دارد.
